Should we call the fire department every time we hear this expression?

Well, "Where there's smoke, there's fire." If it seems like he likes someone else, then maybe he does.

好吧,无风不起浪。如果他似乎喜欢上别人的话,那可能就是事实了。
"Where there's smoke, there's fire" means if there are signs of a problem, then perhaps there is a problem.
“无风不起浪”表示,若有某问题的迹象,那这个问题的确是存在的。
The expression is always about something bad, and often in connection with rumors.
这个表达谈论的一般都是坏事,通常与谣言有关。
If people are talking about a bad situation, there may be some truth to it.
如果人们谈论一个不好的情况,通常可能就是事实。
